Since its first description by Thomas Addison in 1855, dramatic advancements have been made in the management of #Addison's disease, transforming it from a predominantly lethal disease to a manageable condition with in most cases expected long-term survival

A transition from daily oral small molecules to long-acting biologic and nucleic acid-based therapies, including monoclonal antibodies, RNA-targeted agents, immunisation strategies, and gene-editing technologies, delivered subcutaneously or intravenously at extended intervals. Despite this temporal representation, new small molecules and other oral therapies continue to be actively developed, reflecting ongoing innovation across both oral and long-acting treatment platforms.

#Familial #hypercholesterolaemia is an autosomal, semi-dominant condition characterised by markedly elevated plasma #LDL #cholesterol concentration from birth, xanthomas, & early onset of #atherosclerotic #cardiovascular disease (#ASCVD) thelancet.com/journals/lan...
#HeFH #HoFH

#EndoSky #CardioSky

Previous manifestations of ASCVD, especially myocardial infarction and an elevated coronary plaque burden on cardiac CT, indicate a higher risk for ASCVD events. The risk depends not only on very high LDL cholesterol concentrations or exposure to elevated LDL cholesterol during lifetime, but also on the presence or risk biomarkers like male sex, high lipoprotein(a), low HDL cholesterol, high BMI and blood pressure, family history of early ASCVD, smoking and comorbidities like diabetes and chronic kidney disease. Specific familial hypercholesterolaemia risk equations, such as SAFEHEART-RE and the familial hypercholesterolaemia risk score, can integrate this risk. ASCVD risk might also be modulated by the effect of genes unrelated to lipids, as aggregated in a polygenic risk score. LDL cholesterol should always be reduced in patients with familial hypercholesterolaemia. Severe familial hypercholesterolaemia is a clear indication for more intensive LDL cholesterol lowering with PCSK9 inhibitors. ASCVD=atherosclerotic cardiovascular disease.

Turner syndrome is a sex chromosome abnormality in female individuals characterised by the complete or partial loss of one X chromosome in all or some cells www.thelancet.com/journals/lan...
#Turner syndrome

#MedSky #EndoSky

Traditional genotype–phenotype models of Turner syndrome emphasise haploinsufficiency of individual X-linked genes. However, emerging genomic, epigenomic, transcriptomic, proteomic, and metabolomic data suggest that loss of sex-chromosome dosage produces genome-wide regulatory effects. Loss of Xp, PAR1, and X-inactivation escape genes could perturb epigenetic regulators, transcriptional networks, immune pathways, and metabolic signalling. These disturbances propagate across autosomal gene networks and manifest in a tissue-specific manner, contributing to cardiovascular, skeletal, ovarian, metabolic, immune, neurocognitive, and ageing-related features of Turner syndrome. This network model could help explain the limited genotype–phenotype correlation and the marked clinical variability observed even among individuals with 45,X monosomy.
