I think @scarlettmcnally.bsky.social is right in her assessment here.

Promoting walking for everyone is a good thing, but is this really the best way to get everyone more active?

From the @bmj.com back in the summer, but I’m just catching up now.

Excerpt:

NHS England recently announced plans to launch a new incentive scheme that will reward people who achieve 30 minutes of walking a day, adding up to a “marathon in a month.” I see several weaknesses in this plan.

A quarter of the adult population do less than 30 minutes of exercise in a whole week. This is the group who stand to gain the biggest health improvements from any exercise, but they are unlikely to start doing 30 minutes of walking every day. I worry that the new incentive, and the funding, effort, and media outreach behind it, will attract only the people who are active already, further widening health inequalities.

Evidence suggests that incentive schemes lead to minimal long term behaviour change, so this scheme won’t tackle deep rooted cultural and behavioural determinants of inactivity. There is a science to behaviour change. Capability, opportunity, and motivation are required to change behaviour. A third of adults claim that they don’t have the opportunity to be active because of time constraints, safety concerns, or where they live.

The Bristol Declaration deserves more attention. Would be great to see local councils taking it on board, but it would get a real boost if @andyburnham.bsky.social and @heidialexander.bsky.social endorsed it with practical action.

Routes 'Not Designed For Me', heavy traffic and harassment can make cycling feel anything but free for women. This October, glow rides across the UK will shine a light on the need for safer streets for women. Because when it’s safer for women, it’s safer for everyone 🧡

By me (Prof Scarlett McNally)
Record high waiting lists for organ transplantation require awareness and action [EXTRACTS]
I recently competed in the British Transplant Games in Sheffield.1 More than 1000 transplant recipients and live donors...hundreds of volunteers. I’ve always encouraged exercise as part of health, especially for people with health conditions... I’m also encouraging people to register their decisions on organ donation and sign up to be a stem cell donor.2 3
The event...Children whose organs had been failing were running, laughing, and thriving...
There was a standing ovation as donor families entered for the opening ceremony...powerful stories...people determined to make something positive from the grief of losing a loved one, by allowing organ tissue donation. Living donors now make up 30% of kidney transplants.4
More than 8000 people are now waiting for an organ transplant in the UK, with median waits of one to two years.4 Although all UK nations now have assumed consent,5 fewer organs are made available than are needed.4 Unfortunately...
The UK is falling behind other countries on transplantation, and the number of operations hasn’t increased in three decades.6 A BBC investigation found...insufficient investment, and an exodus of senior transplantation surgeons from the NHS.6 , fragmented systems causing last minute cancellations, and outdated technology that creates challenges in assessing the viability of donated organs.7
Two reports: need for investment in modern technologies, development and retention of a sustainable transplantation workforce and data/coordination across centres.8 9...
My own transplant was an autologous stem cell transplant, that replaced some of my own stem cells after myeloablative chemotherapy for myeloma and cardiac amyloidosis.11 12
