Tested against placebo, in light blue: Semaglutide injection against placebo (SELECT, 17,604 patients): 0.80, interval 0.72 to 0.90. Semaglutide pill against placebo (SOUL, 9,650 patients): 0.86, interval 0.77 to 0.96. Tested against another drug, in purple: Tirzepatide against dulaglutide (SURPASS-CVOT, 13,299 patients): 0.92, interval 0.83 to 1.01. Orforglipron pill against insulin glargine (ACHIEVE-4, 2,749 patients): 0.77, interval 0.52 to 1.13. Orforglipron's estimate lies among the others, but its interval is about 4 times as wide and crosses 1: its trial was sized to rule out harm, which it did, not to prove a benefit. These are separate trials, not head-to-head comparisons. Dulaglutide is itself a heart-protective drug, so a ratio against it cannot be set beside a ratio against placebo, and SELECT enrolled people without diabetes while the other three enrolled people with type 2 diabetes. For orforglipron this is the three-part endpoint; its primary endpoint, which also counts hospitalisation for unstable angina, was 0.84 with an interval of 0.59 to 1.20.

1/3 of T1D adults are misdiagnosed as T1D and while thry do not go into DKA as much as children, they progress more slowly with higher c-peptide levels giving more opportunity for intervention e.g. delaying insulin need by two years with Teplizumab. #EASD2026 #dedoc⁰

A study looking at smart insulin pens and how people responding to missed bolus alerts do better. However the best TIR was still 60%, so inferior to AID. Choice is good but I'll personally stick with my pump. #EASD2026 #dedoc⁰
