More than one in 10 (10.1%) of patients who attended a major A&E department in England last month waited 12 or more hours before being discharged, admitted or transferred.

That's 157,574 people waiting over a half a day in just a single month.

🏆 At RCEM Annual Conference 2026, Ryan McHenry received the Rod Little Prize for exploring whether people living in more deprived areas are more likely to be redirected away from Emergency Departments and then need to return for care.

How fair is emergency care?
Q&A with Ryan McHenry,
Rod Little Prize winnerWhat initially sparked your interest in this area of Emergency Medicine?
It probably started with my first job in Glasgow Royal Infirmary, working on a respiratory ward in a pocket of social deprivation that is amongst the worst in Europe and where it was so clear that the reason that almost all of the patients were there due to the social circumstances through which they had lived ... that led me to wondering about ways we could help, and a bigger programme of work surrounding health inequalities in emergency care.
How did it feel to hear your name announced for the Rod Little Prize?
After I got over the shock that it had been my name that had been read out, it was a lovely experience to receive the prize from HRH The Princess Royal. We had a nice chat about the work and its relevance to emergency medicine. But I was acutely aware that we were speaking, off mic, in front of a few thousand people, so it made the whole experience seem a little more surreal.
We know that the predominant cause of crowding in emergency care is exit block, but that’s been difficult for health systems to overcome, so instead they look towards other areas, maybe ‘quick wins’ of demand management. But these strategies are often implemented without much understanding of the populations that are reached, or about how they might serve vulnerable groups.
Why is it important for Emergency Medicine clinicians to think about health inequalities when looking at crowding and demand management?
