In Amazonian healing the main tool is song. The icaro is said to be received from a plant during a long apprenticeship, sung with mapacho smoke, a rattle or a leaf fan. Ethnographers describe them; some are not meant to be recorded.

The youngest of the three words was proposed in 1979 by a small group of scholars: entheogen, from Greek entheos, having the divine within. It names a use, not a chemistry. I use it mainly to report how a tradition understands its own practice.

One detail from the anthropological record turns our model of medicine around. In many shamanic settings the healer takes the plant, not the patient, working on the sufferer's behalf. The sufferer may never touch it. Modern trials assume the reverse.
